Why Legacy Living Placement?
We offer placement services that are at no cost to families or their loved ones. Both Judy and Kim understand both personally and professionally how hard this process is for families and their loved ones. We strive to provide information, comfort, and support throughout the entire process. We can place directly from the hospital, skilled nursing, from the family home, or from the individual’s home. Most people wait for a medical crisis or fall before considering supervised care but being proactive may reduce the chances of a medical crisis or falls. Stabilization and safety is our priority for a care environment.
Our Process:
- Phone call for initial assessment
- When appropriate, meet with family, your loved one, and/or the individual who is in need of care
- In-depth needs, wants, and desires assessment
- Establish time frame and develop a plan
- Schedule tours
- Accompany family or individual on tours
- Once a community or home is chosen we connect the community with the family or loved one to begin the transition process
- Follow-up to insure that placement is going well
How do I Know it is Time to Start Looking for a Higher Level of Care for My Parents or Myself?
- An injury or non-injury fall may be a sign that it is time to look for 24/7 supervised care.
- If you or your loved one has multiple hospital admissions or emergency room visits, it is likely time for 24/7 care.
- If you are concerned about your loved ones memory and their ability to safely get through their day.
- If you are concerned about potential falls or a medical crisis, 24-hour care can often delay or avoid a major crisis.
- If you or your aging loved ones home has multiple levels in the home, it may be time to start exploring 24/7 care.
- If you are the primary caregiver for your loved one and your physical and mental health is starting to suffer.
